Harbors Home Health & Hospice is committed to providing compassionate, high quality care to patients in their homes. 

The Patient Care Scheduler is responsible for coordinating and scheduling patient visits for home health and hospice services.  This role ensures timely and efficient care delivery by managing clinician schedules, patient needs and compliance requirements.

Key Responsibilities
Schedule patient visits for nurses, therapists or other care team members
Communicate with patients, families, and clinicians to confirm appointments
Maintain accurate records in the scheduling system and electronic health records
Monitor and adjust schedules to accommodate changes in patient condition or staff availability
Ensure compliance with agency policies, state regulations, and payer requirements
Provide excellent customer service and timely follow-up on inquires

Previous experience in healthcare scheduling, home health or hospice strongly preferred
Strong orgnizational and time-management skills
Proficient in scheduling software and EHR systems
Excellent communication and problem-solving abilities
